Bottom, as I recall (we are talking about 17 years ago, so, memory is somewhat less than reliable).

Became a secondary infection - headed down my throat along a blood vessel. My boyfriend of the time was told to stay up with me and make sure my breathing stayed regular while I was sleeping, as there was some concern that the swelling from the infection might push hard enough (umm...isn't that cartiledge - that's some serious infection, methinks)
